The study evaluates the four major existing community extension programs in tertiary level of Notre Dame of Dadiangas College. Based on the results of the evaluation, a program Model of a Community Extension Program in Educational Institution is conceptualized. The results reveal that the objectives of the four programs are relevant, clear and attainable. The staff handling the program are qualified and competent. On the whole, the program is successfully implemented and possible to institutionalize the program in all levels of the institution.
